For New Patients

New patients should call the office at 305.650.1414 to schedule a New Patient appointment. You will be asked to provide some personal information for identification purposes as well as an email address to save time with new patient forms on the day of your visit! Once an appointment is made you will receive an email with instructions on how to fill out our new patient forms electronically saving your precious time in the office!

Follow-Up Appointments:

If the doctor requests that you return for a follow-up visit, please be sure to make your return appointment before leaving our office or within a week following your original appointment. This will ensure your appointment is at a time that is convenient for you.
