National Physician Transparency Payment Program
The National Physician Transparency Payment Program, often called the “Sunshine Act” was developed to promote transparency in the financial relationships with healthcare providers and health care manufacturing companies in the United States. “These relationships can include money for research activities, gifts, speaking fees, meals, or travel. The Affordable Care Act requires CMS to collect information from applicable manufacturers and group purchasing organizations in order to report information about their financial relationships with physicians and hospitals. Open Payments is the federally run program that collects the information about these financial relationships and makes it available to you.” Due to his dedication to the improvement of patient outcomes and advancement of spine surgery, Dr. Garcia is often sought by the medical device industry as a consultant, educator, active investigator and researcher.

All forms (ie. FMLA, insurance, US Dept of Labor, etc.) will be completed within 3-5 days of the date of receipt. Charge for this service is $50/form.
In order to maintain consistent patient flow and to minimize wait time, patients are required to inform our staff of any rescheduling or cancellations with 24 hours advanced notice to avoid a no show fee of $25.
X-ray copies are available upon request at the time of your appointment. Charge for the service is $10 per X-ray due to supplies cost.
Consulting and Investment Disclosures:
Disclosures: Dr. Rolando Garcia, Jr. is an active investigator and researcher involved in research and collaboration with implant manufacturers. As such, some, none, or all of the implants utilized in your case may be implants manufactured by companies with which Dr. Garcia has consulting and/or royalty agreements. In addition, your procedure may be performed at an outpatient or inpatient facility in which Dr. Garcia may have fractional ownership and/or investment interest. If you would like more detailed information please feel free to ask.
Telemedicine Disclosures:
Telehealth to be provided in a private setting, according to HIPAA safeguards to limit incidental uses or disclosures of protected health information (PHI). Such reasonable precautions may include using lowered voices, not using speakerphone, or recommending that the patient move to a reasonable distance from others when discussing PHI. Intake form may be sent to patient to obtain consent before the virtual appointment. The cost of the telemedicine request is non-refundable, although the patient can reschedule for a later date/time with at least 24 hours in advance.
